  • Carefully document all medications, diagnoses, treatments and consultations
  • Diagnose and provide appropriate dental treatment to patients, including regular cleanings, root canals, Extractions could include simple/ surgical/ full mouth
  • Active New York state dental License
  • Active CSR (if applicable)
  • Active DEA License
  • DDS/DMD
